 Here's a good format:

Gender and animal
Body type (athletic, average, ect.)
Hair length, style, and color
Fur pattern,
Little details, such as human feet/paws, eye color. Stuff like that.
(And for female characters I have to ask to get it right. Chest size)
Clothing/jewelry (if any.)
